RevUp Care Management is a service provided by your doctor to help you better manage your health and stay connected between office visits. The RevUp app is a companion to this service that allows you to communicate with your health coach, set and track your health goals, and easily log your health vitals. - Communicate with your health coach - Set your personal health goal - Easily track your health vitals and see history of vital tracking - See the care plan your health coach has created for you - Read health tips related to your health conditions - Notifications for logging reminders and new messages - Manage communication settings You can also integrate to your health devices through Apple’s HealthKit. This will connect your health information from your iOS device with your RevUp Care app.
