Description
The trial will pilot test the school-partnered collaborative care (SPACE) model compared to usual school-based diabetes care for children with type 1 diabetes ages 6-12 years. SPACE will connect caregivers (e.g., diabetes provider, family, school) on a shared treatment plan for children with type 1 diabetes. Note: Participation in this trial is by invitation only.
