CCP Cares

As a Community Care Plan member, you have easy and convenient access to CCP Cares, our member portal anywhere they go at any time. Our secure self-service portal lets members get personalized information about their health plan benefits and services, as well as health information in their preferred language (English and Spanish). Once you sign into with your confidential username and password, you can: View: • You or Your Child’s virtual member ID card • Alerts and reminders • Coverage and benefits • Authorization and referral status • You or Your Child’s Explanation of benefits Search for: • Doctors and providers • How much services may cost you • Health information in our comprehensive health library Complete Tasks Like: • Change your or your child’s Primary Care Physician • Complete questionnaires and surveys, like our Health Risk Assessment (HRA) • Reset your password • Upload health and fitness data, including data from the “Apple health app,” when enrolled in self-tracking programs. Members can also personalize the portal by using Touch ID to login, choosing a profile nickname and what they want to see on their menu like favorites and shortcuts.
