Restricted to HWS Staff Health Wealth Safe Care is a remote patient monitoring app (RPM) that the care managers and practitioners participating in the program can use to provide remote health monitoring and tele-health services to patients. This application allows the care managers and practitioners track patient health and engage with patients. Patient engagement and monitoring significantly improves patient wellness, prevents ER visits, and reduces hospitalizations.
