Description
MyNukaConnect is an application that tracks the data collected by an individual’s
remote monitoring device(s) (Smart watch, Bluetooth pairing weight scales, BP
cuffs etc.) and can be shared with their Integrated Care Team at Southcentral Foundation. It also allows the sharing of information from HealthKit to a Southcentral Foundation Integrated Care Team. The purpose of this
application is to support a healthy lifestyle and easily communicate health trends
with the user and their Integrated Care Team.
