The Southlake Health Navigator serves as a single virtual front door for patients and families to conveniently discover health information and services in the region. Through the Southlake Health Navigator, you can create an Ontario trusted account and use this verified identity to securely manage your hospital appointments, access general health and service information, and access your personal health information as well as virtual services. Over time, additional resources and digital services will be added. As a digital tool that is designed by patients for patients, we encourage you to share your feedback and suggestions to enrich your experience with the app and for continuous quality improvement.
