The Aliviado app is focused on helping you to care for persons living with dementia. It contains tools, resources, and other content for both clinicians and caregivers. The Aliviado app comes from Aliviado Health, an arm of the Hartford Institute for Geriatric Nursing at the NYU Rory Meyers College of Nursing focused on helping care organizations and individual clinicians and caregivers provide high quality dementia care through better symptom management, and increasing the quality of life for persons living with dementia and their caregivers.
Thousands of clinicians are enrolled in the Aliviado Health educational program, in which they have access to innovative training, education, mentoring, and resources, giving them advanced expertise in the complexities of care for persons living with dementia. Aliviado Health's tailored, compassionate care approach helps patients and caregivers effectively manage symptoms, resulting in improved quality of life, decreased hospital re-admissions, lower healthcare utilization and medication use, increased staff knowledge and confidence, reduced costs, and increased patient satisfaction scores.
With the Aliviado app, you can have access to a portion of these resources focused on care for older patients with dementia, including caregiver educational materials. This includes:
- Assessment Instruments: Through a series of questions, assess the severity of common symptoms that accompany dementia such as pain, depression, delirium, behavioral issues, as well as caregiver stress.
- Care Plans: Once symptoms have been confirmed, Care Plans allow proper management of the symptoms.
- Educational articles for caregivers.
- News and blog articles about caring for older patients with dementia.
- Ability to send Aliviado feedback.
