Certify Care

The iPad version is designed as as a solution enabling patients to move around or fill the forms from their seated location. The app is accessed through DOB and Username for a particular patient who created their appointment with a Medical staff within the HealthCare Practice using the CARE portal.  The app also comes with a time out option which logs the patient out in case of inactivity thereby securing patient information. For returning patients, upon authorization, the app also displays information allowing the patients to edit or make changes before submitting the forms. Certify Care app facilitates users to fill the patient intake forms and consent forms to speed up the appointment process.
